At the top is a drawing of a man, head only, who is holding in his left hand, his cowboy hat. He is also saying Howdy Folks! I'm Dick from Dickinson. Below this are the words HOSPITAL BIRTH CERTIFICATE. Below this it reads This Certifies that Dick from Dickinson was born in St. Joseph's Hospital of Dickinson, North Dakota on the tenth day of September A.D. 1953. In Witness Whereof the said Hospital has caused this Certificate to be signed by its duly authorized officer and its Corporate Seal to be affixed. Doctor (signature) R. Rodgers, Hospital No. (none given), Superintendent, (signature) Sister Fredegard R.N. In the left hand corner of the certificate is the seal of the Hospital.
